1. What this website is

This site is a business information site describing the physician-office service lines operated by JET Anesthesia LLC, plus a contact channel for scheduling and administrative inquiries. It is not a medical device, not a patient portal, and not a source of clinical guidance.

2. No medical advice — the boundary that governs everything

Nothing on this website is medical advice. Descriptions of service lines (for example "cardiologists' offices" or "psychiatrists' offices") are classifications of business activity, not recommendations, diagnoses, or treatment information. You agree that:

• reading this site creates no physician–patient relationship, no clinician–client relationship, and no duty of care from JET Anesthesia LLC to you;
• clinical decisions are made only by licensed clinicians in the setting of an actual appointment;
• for emergencies you will call 911 or go to the nearest emergency department rather than contacting this site;
• you will not rely on any page here to make, defer, or avoid a healthcare decision.

3. No engagement by browsing

Submitting the visit-request form or sending an email does not guarantee an appointment, a response time, or acceptance as a patient by any office. A service relationship begins only when the relevant office confirms it through its own intake process.

4. Acceptable use

You agree not to:

• submit false, fraudulent, or another person's information through the forms;
• send protected health information of third parties through unsecured site channels;
• probe, scan, overload, or attempt to disrupt the site or its hosting;
• scrape the site in a manner that degrades service or copies content in bulk;
• misrepresent an affiliation with JET Anesthesia LLC or any physician;
• use the site for any unlawful purpose under Utah or United States law.

5. Accuracy of the chart

The fifty-one service lines on the Service Wards page describe our business scope as classified at publication. Individual office availability, hours, and which lines are actively seeing patients may change; the site is updated periodically but is not a real-time availability board. We may correct, renumber or restate lines without notice.

6. Intellectual property

All content on this site — the "Monitor" design system, the workstation and ward graphics, page copy, and the JET Anesthesia name and marks — is owned by or licensed to JET Anesthesia LLC and protected by United States intellectual-property law. You may view and print pages for personal, non-commercial reference. Any other reproduction, redistribution or derivative use requires our prior written permission.

7. Third-party resources

The site loads fonts from Google Fonts and is hosted on Google Firebase; outbound links, if any appear, lead to sites we do not control. We are not responsible for third-party content, policies or availability.

8. Privacy

Your use of the site is also governed by our Privacy Policy, which explains what little data the website collects and how it is handled. Do not send clinical information through the site; see Section 4 of that policy.

9. Disclaimer of warranties

The site is provided "as is" and "as available". To the maximum extent permitted by law we disclaim all warranties, express or implied, including merchantability, fitness for a particular purpose, and non-infringement. We do not warrant that the site will be uninterrupted, error-free, or that descriptions of service lines reflect current scheduling availability.

10. Limitation of liability

To the fullest extent permitted by law, JET Anesthesia LLC, its members, managers, staff and developer shall not be liable for any indirect, incidental, consequential, special or punitive damages, or for lost profits, data, goodwill or health outcomes, arising from or related to your use of — or inability to use — this website. Where liability cannot be excluded, our aggregate liability for all claims relating to the website shall not exceed US $100. This section does not limit liability that cannot lawfully be limited, and it does not address — or limit — any liability arising from actual clinical care, which is governed by separate agreements and law.

11. Indemnification

You agree to indemnify and hold harmless JET Anesthesia LLC from claims, losses and expenses (including reasonable attorneys' fees) arising from your breach of these terms, your misuse of the site, or your submission of another person's information without authorization.

12. Governing law and venue

These terms are governed by the laws of the State of Utah, without regard to conflict-of-laws rules. Any dispute arising from this website shall be brought exclusively in the state or federal courts located in Salt Lake County, Utah, and you consent to their jurisdiction.

13. Changes

We may revise these terms from time to time; the current version is always posted at /terms-of-service. Continued use of the site after changes take effect constitutes acceptance.

14. Severability

If any provision of these terms is held unenforceable, the remainder continues in full force.
