1. What this policy covers — and what it does not

This policy governs information handled through this website: pages you read, forms you submit, and messages you send us by email or phone. It is a website privacy policy, not a Notice of Privacy Practices under HIPAA. Clinical records created inside an actual medical office are governed by that office's own privacy practices, its consent forms, and applicable health-privacy law — documents you will receive if and when you become a patient of a specific practice.

Nothing submitted through this website becomes part of a medical record, and this website is not a patient portal.

2. Information we collect

2.1 Information you give us

• Visit-request details — your name, email address, optional phone number, chosen ward, and the free-text message you submit through FORM JET-REF.
• Correspondence — the contents and metadata of emails you send to reach.clara.bennett@jetanes.shop and notes of phone conversations with the intake desk.
• Business inquiries — company name, role and contact details if you contact us about records, partnerships or administrative matters.

2.2 Information collected automatically

• Server logs — IP address, browser type, referring page and timestamps, recorded by our hosting provider for security and reliability purposes.
• No advertising trackers — this site does not run advertising networks, social-media pixels, or behavioral-analytics profiles. We load fonts from Google Fonts; Google may process your IP address to serve the files.

3. How we use information

• Routing your request to the correct ward or service line and responding to you;
• Scheduling-related correspondence and business replies;
• Operating, securing and debugging the website;
• Meeting legal, accounting and record-keeping obligations under Utah and federal law.

We do not sell personal information, do not share it for third-party marketing, and do not build advertising profiles.

4. What not to send us

Please do not submit diagnoses, medication lists, symptoms requiring urgent attention, medical-record numbers, insurance ID scans or other protected health information through the website form or ordinary email. The intake desk will direct you to the appropriate secure channel if a clinical relationship is established. If you believe you are experiencing a medical emergency, call 911.

5. Cookies and local storage

This site is intentionally minimal: it sets no tracking cookies and stores no persistent identifiers in your browser. Session-level technical storage may be used transiently by your browser to render pages. You can block or clear such storage in your browser settings without losing access to any page on this site.

6. Who sees your information

• Our intake staff, on a need-to-know basis, to answer your request;
• Infrastructure providers — our hosting provider (Google Firebase) and email carriers process data to deliver the site and messages;
• Authorities, where disclosure is required by law, court order, or to protect the safety of a person.

7. Retention

Visit-request messages and correspondence are retained for up to 24 months so we can follow threads and maintain business records, then deleted or anonymized unless a longer period is required by law. Server logs are rotated by our hosting provider on their standard schedule.

8. Security

The site is served exclusively over HTTPS. Access to inbound correspondence is limited to named staff on managed devices. No method of transmission or storage is perfectly secure; if we become aware of a breach affecting your information, we will act as required by Utah law.

9. Your choices and rights

• Access & correction — ask us what website-submitted information we hold about you and request corrections;
• Deletion — request deletion of your website-submitted information, subject to legal retention duties;
• Opt-out of replies — tell us at any time to stop corresponding, and we will.

10. Children

This website is a business site directed at adults and is not intended for children under 13. We do not knowingly collect children's data through the website; healthcare services for minors are arranged through the offices with a parent or legal guardian.

We may update this policy; the current version will always live at /privacy-policy on this domain. Material changes will be described at the top of the page.
